Neuralink released a short video of about 23 seconds, showing VOICE trial participant Kenneth Shock using an implanted wireless N1 device to decode neural signals of intended speech into synthesized voice, telling his wife "I love you." The company captioned it "a beautiful surprise," which Elon Musk retweeted. The device is still in trials and has not received general approval from the U.S. Food and Drug Administration; results vary by individual and session.

Shock was diagnosed with amyotrophic lateral sclerosis in 2024, gradually losing his ability to speak, and received the implant in January 2026, becoming the second participant in the VOICE project. The system reads signals directed at the tongue and throat, identifies phonemes, and reconstructs words, attempting to restore pre-illness vocal quality, which his wife calls "the original Ken." A longer demonstration in March showed him saying, "I’m talking to you with my mind."

This differs from the Telepathy approach, which helps high-level paralyzed individuals control cursors and devices with their thoughts; VOICE targets aphasia, turning intended speech into audible voice. Previous lab examples include a 2021 University of California, San Francisco project translating brain activity into sentences, a 2023 Stanford team achieving about 62 words per minute, and real-time internal speech decoding by 2025, though unstable. Neuralink emphasizes embedding lab capabilities into a usable wireless implant rather than proving scientific principles for the first time.

At the same time, Paradromics announced its first Connexus implant recipient could answer open questions in real-time with voice and text, communicate with grandchildren, and distinguish neural patterns between attempting to speak and merely imagining speech. The Neuralink video did not provide word speed or error rates, noting that results "may not represent all participants or future outcomes."

In market mechanisms, this is emotional validation preceding reimbursement coding: what is purchased is the trial slot and subsequent subscription for families of aphasia patients willing to pay to "hear that person's voice again," while the pricing treats brain-computer interfaces as still science fiction or game controllers. Beneficiaries are implant companies racing for VOICE indications; those under pressure are academic devices that can only measure once a week in the lab and cannot produce vocal quality as a product. Before regulatory approval, the video serves as a fundraising and enrollment tool, not a sales license.

Additionally, what is decoded is the cortical activity related to intended speech, not random thoughts; open dialogue, continuous speech rate, and stability in non-home environments have yet to be quantified in this short video.
