The U.S. faces a simultaneous shortage of doctors and high barriers to becoming one, which are not contradictory. The most visible bottleneck at medical schools is the residency positions and funding.

According to the Association of American Medical Colleges, the number of applicants for MD programs increased by 5.3% for the 2025-2026 academic year, while first-year enrollment only rose by 1.2%. The total number of enrolled students has surpassed 100,000 for the first time. The visible barriers include scores, interviews, and rejection letters, with emotions heavily weighed down here.

Getting through medical school does not equate to being able to practice independently. Residency training typically lasts three to nine years, with an additional one to three years for subspecialties, during which hospitals cover salaries, benefits, and training costs. In the 2026 residency match, the matching rate for U.S. MD graduates was 93.5%. However, about 9,696 applicants remained unmatched, while 2,862 positions were unfilled across 941 programs. This simultaneous occurrence of people having nowhere to go and positions going unfilled is evident.

Federal Medicare funding for residency positions at most teaching hospitals is still capped at the training levels from around 1996. A law passed in 2021 added 1,000 full-time equivalent positions to be phased in over several years, but this does not mean an immediate increase of 1,000 trained doctors each year. Hospitals can use their own funds to exceed training limits, but expansion will not automatically occur with an increase in applicants. The Health Resources and Services Administration's model predicts that by December 2025, there could be a shortage of approximately 141,160 full-time equivalent doctors by 2038; urban supply rates are about 95%, while rural areas are around 42%.

International medical graduates now make up about a quarter of the U.S. physician workforce, but their qualifications often do not automatically transfer. Some states will allow eligible internationally trained doctors to obtain full licenses without completing the usual North American post-graduate training starting in 2026, subject to additional years of practice, exams, designated employers, or service areas, but the effectiveness of this pathway remains to be seen. A 2025 survey by the American Medical Association found that managing prior authorizations takes an average of about 13 hours per week for both the physician and staff.

The push for expanding federal residency positions, rural service, and license reciprocity is the policy capital; the narrative that blames the shortage solely on "medical school being too hard to enter" is the counter-narrative. The driving force is the mismatch between the increase in applications and the formula for residency positions, revealed by a set of official numbers. Beneficiaries include teaching hospitals that can secure training funds and projects in shortage areas; those under pressure include non-urban primary care and practicing doctors whose outpatient time has been consumed by administrative processes.

The real market transaction is for training positions and outpatient hours, not the number of applicants.
